I am pleased to make the Agile Survey available (again). This online survey allows you to assess your team’s agility. It was originally designed as a checklists for teams that were converting to agile. It can also be used to identify a team’s current level of agility and chart their progress if the survey is periodically repeated. I have used this several times over the year.

You review the survey as a PDF here AgileSurveyV3, you can also use the lss version (zipped) and import into a locally hosted version of LimeSurvey (an OpenSource survey tool), this will allow you to tweak the survey to your particular needs.
